I'd like to buy a spare transmitter but before I drop $50 for one, can I just plug and play or does it need some kind of programming.

Yep, sensors need to be programmed. Only 4 at a time, so a fifth sensor needs to be programmed at the dealer once the wheel is put on.
It only take a minute to program, but its another trip to the dealer.

Is this for the spare? I wonder if the spare already has a sensor on it??

yes it's for the spare. I know there isn't one because it has a full rubber stem unlike the 4 on the Mada'

I have aftermarket 22" wheels and I'm going to buy another matching wheel and tire for the spare.

Does it have to be mounted to be programmed?

The spare does not come with a sensor, and adding one may cause a problem. If you have the Nav package it reads out pressure on each on the four tires on the ground. The fifth sensor may confuse things.
